The Newman Breastfeeding Clinic requires two separate referral forms: one for the breastfeeding parent and one for the baby (or babies, in the case of multiples). Referrals from either a doctor or midwife or nurse practitioners are acceptable.
Doctors/Midwives/Nurse Practitioners:
To refer your patient, print the forms and fax them to 416-352-1224 or 416-498-0012.
Patients must book their own appointment.
Referrals must be received before we can provide your patient with an appointment date.
Referrals forms must have doctors printed name and billing number in order to be processed and complete.
